I think the key difference is whether the messiness is selective. If every line is rough, the dialogue can feel merely unedited; if only the important moments are interrupted, repeated, or slightly unfinished, then it starts to resemble actual speech. A real conversation is full of false starts, but it also has rhythm, and good novel dialogue usually keeps that rhythm even when the surface is irregular. What breaks it for me is not neatness itself, but dialogue that sounds as though it exists only to deliver information.
